How do you fill your report cards?

In my school we get the students score on writing, reading, listening and speaking through homeworks, lessons, act in classroom and a final exam. But every skill has different features that in my opinion can not be measured or graded in the same way. For example if I make a question about grammar, when my students respond, I think their answers should be considered as a point on speaking act in classroom but not writing even if we have been told about grammar rules. How do you grade Speaking homeworks? does exist something like that? How do you grade act in classroom on Listening? Please I will be so grateful if you give me comments and bibliography I could consult. And a last favor, can you share with me models of report cards you use in your institutions? You would better focus on flency regarding learners' level which may contain the idea, situation, stress and intonation and also the use of grammar but totally we are looking for successful communications in speaking, so the result in conveying what one means is more prior to how grammatically correct, he speaks.
As we divide speaking to both productive and interactive, you may find it easier to plan a tour-together outdoor workshop where your students are missioned to use English on a purpose and it will be more obsevable to evaluate them in a real life situation. But if its not that much easy, so let them make the situation in class and go for themseves moire freely but planned.
Remember in a communication there is always a GAP, which makes our dialogs valid and senseful.
They should be provided with a need to speak.

I think this is a good question,because I have the same problem with listening.I teach Top Notch to Begginer and Elementary students and as you know the listening exercises are not too many and I always have problem scoring listening part for students in their class participation which I should include in their report card.
